A supplement is often treated as separate from medication because it is bought without a prescription. The body does not make that distinction, and both are handled by the same set of biochemical systems.
The same enzymes process both
Most drugs are metabolised in the liver by cytochrome P450 enzymes, a family that also handles many plant compounds found in botanical supplements.
A substance that increases the production of one of these enzymes causes drugs processed by it to be broken down faster, lowering the concentration reaching the bloodstream.
A substance that inhibits the enzyme has the opposite effect, allowing the drug to accumulate to higher levels than the prescribed dose was intended to produce.
Absorption can be blocked in the gut
Some interactions happen before anything is absorbed. Minerals such as calcium, magnesium, iron and zinc can bind certain drug molecules in the intestine, forming complexes that are not absorbed.
The mechanism is chemical rather than pharmacological, and the practical consequence is that a drug can be rendered largely ineffective by a mineral taken at the same time.
Separating the timing addresses some of these, which is why instructions occasionally specify an interval rather than avoidance.
Some interactions add effects rather than block them
Where a supplement and a drug act on the same physiological process, their effects can combine.
Substances affecting blood clotting, blood glucose, blood pressure or sedation are the common examples, since each of these is also a target of widely prescribed medicines.
The result is not a failure of either product but an exaggerated version of an intended effect, which can be as consequential as an absent one.
Narrow therapeutic range makes interactions serious
Many drugs tolerate a fair amount of variation in blood level without a change in outcome.
Others have a narrow window between an ineffective concentration and a harmful one, and for these a modest shift caused by anything else being taken is significant.
Anticoagulants, some heart rhythm drugs, transplant medicines and certain epilepsy treatments fall into this group, which is why they attract specific interaction warnings.
Disclosure is the practical step
Interactions cannot be worked out from a label, because the label describes the product alone and not the combination.
Only a pharmacist or doctor with the full list of what someone takes can check for known interactions, including products that seem too ordinary to mention.
That list should include vitamins, botanical products and anything taken occasionally, and it is worth raising before starting something new rather than afterwards.
